You might be surprised to know that when I was diagnosed with type one diabetes eight years ago, I didn’t go on a continuous glucose monitor right away (Gasp!). Yep, it’s true. At the time, I had just gotten on an insulin pump and honestly didn’t want to have a second device on my body.
Since then, we’ve seen a lot of advancements in technology. From Control IQ, to Basal IQ to now the Omnipod 5 coming on its way, there are a lot of ways that life with diabetes is slowly becoming easier. At the end of the day, the less mental energy we have to put into diabetes, the more energy we can put into places we actually want to. However, with each new advancement we see our clients slowly integrating new pumps and CGMs – and with each new piece of technology, we see that there are faults in it.
False Expectations:
As a community, we’ve been awaiting automated technology for a long time. These systems are built up to seem like they can do it all. They prevent highs by increasing basal or giving us corrections. They prevent lows by suspending our basal. It sounds great, but is it too good to be true?
We see it time and time again. Patients are built up with an expectation that the pump will increase their time in range and lower their A1C when in reality, it’s adding to the frustration. Instead of seeing smoother lines and more time in range, they see increased fluctuation. We’ve seen it in our own clients.
The Technology is Only As Strong As Your Decision Making:
In reality, the new use of this technology doesn’t take away the need for decision making in our day to day with diabetes. It may look different than when we were on manual mode, but nonetheless it requires a transition period.
The technology can only work optimally for us if:
Our baseline settings (basal rates, carb ratios and correction factors) are accurate.
Our decision making works with the technology – instead of against it.
When working with automated technology that changes our doses based on an algorithm, we need to remember that it doe not know the timing of our meals, the macronutrient make up of our meals, how we decide to treat our lows and so-forth.
How Can We Dial In?
Our best chances at working with the technology come down to dialing into our behavioral and management baselines. Just like anything else, you need the foundations in place to build up. Think of it like working our muscles. If we work out and use the incorrect form, we’re going to grow the wrong muscles.
